What creates a derby?

Common ingredients include clubs from the same city, nearby towns, neighboring regions or a long-running sporting dispute. Some rivalries are called derbies even when the teams no longer share the same division.

Is a derby a tournament?

No. Derby is a description of a fixture or rivalry, not a separate competition. League, cup and playoff rules still govern the match.
